Description of Services Provided by AHTFY

• Comprehensive assessment of the client’s physical, cognitive, and medical care needs.
• Matching the right communities to our clients’ care needs, personal preferences, and financial ability.
• Touring these communities with our client and sharing additional information and perspectives based on our personal experience.
• Supporting our client after the move (care conference, communication with the community, problem-solving, etc.).
• Recommending, based on our first-hand knowledge and experience, any additional supporting services that you may need (legal, case management, downsizing and moving, private caregivers, etc.).
• Collaborating with other placement agencies from out of town/state/ if needed.

We ask that you or anyone in your family NOT contact any care community on your own. If you wish to visit a care community not on our recommendation list, please let us know, and we will arrange the tour. This will ensure our service is free to you.

The final decision and choice of care community are made by you, our client, or the client’s responsible party (family member, agent under a valid power of attorney, a guardian, or conservator). The role of AHTFY is that of a consultant, not a decision-maker.

When a choice is made, you and the community will discuss the financial deposit and community move-in procedures. The community will conduct an assessment, as required by its rules and regulations, before accepting clients.

Any questions or concerns related to the community of your choice and their services to you or your loved one can be addressed with us at any time.

We may advise you, if necessary, to seek an additional outside professional service such as legal, financial, or medical. AHTFY does not receive or pay any fees to or from those service providers.

Care Communities with Which We Contract

• AHTFY contracts with the following types of care communities in Lane County: Independent Senior-Living Communities (IL), Assisted Living Facilities (ALF), Adult Foster Homes (AFH), Residential Care Facilities (RCF), and Memory Care Facilities (MC).
• AHTFY contracts with private-pay and Medicaid-contracted communities.
• We regularly visit and review the care communities we contract with in person to ensure the quality of services they provide in our referral network. Care communities’ complaint history can be found at www.ltclicensing.oregon.gov
• AHTFY has no ownership interest in any of the care communities in Lane County.

Referral Fee

In most cases, the care community of your choice compensates AHTFY for our service to you*

Our compensation is a percentage of your full one-month pay to the care community. The fee has no expiration date, regardless of move-in time.

There are a few exceptions when the care community does not pay the fee for our services to the client. We will disclose any fee the client may be responsible for after completing the initial free assessment.

Privacy Policy

• The personal and medical information we collect helps us understand your medical and physical care needs, cognition, and budget.
• We will collect that information only with your full consent.
• This information will be shared only with the communities we will be touring.
• We will only retain this information as long as necessary to fulfill those purposes.
• We do not sell your personal information.

Termination of Service

The client can terminate our services and the use of the information we collected at any time, with or without cause. You agree to notify AHTFY, in writing or by phone, of the decision to terminate services. Once services are terminated, we will not contact you or use your information per your request.

AHTFY reserves the right to terminate service to the client with written notice in situations such as, but not limited to, refusal to cooperate or any fact or circumstance that arises that would render services unethical, unlawful, or ineffective.

Your signature or email confirmation is an acknowledgment that you have received, read, and understood this information.

With your signature or email confirmation, you authorize AHTFY to share the client’s information with the facilities to which the client will be referred.
